Current registered USA Water Polo clubs are covered under the USA Water Polo general liability insurance policy provided that ALL participants (athletes, coaches, & referees) are current members of USA Water Polo. Club practice insurance covers participation for 1-2 clubs. NOTE: If more than 2 club teams are participating in scrimmage/event; an event sanction must be completed.

* In order for this policy to be active, all athletes and coaches participating in club practices must be current USAWP members. Roster program available to ensure event participants are all active members.
How can I obtain a copy of my club practice general liability insurance?
If your pool facility requires a certificate of insurance, the club administrator can log in to his/her personal USAWP Membership account and renew the club by looking within the "Club Admin Area" tab on the left column and clicking on the "Practice Insurance" option.
* If you are not sure of who the club administrator(s) are for your club, contact the National Office by phone (714) 500-5445 or by email: firstname.lastname@example.org. In order to be added as an administrator, the current primary contact of the club must send an email to email@example.com to add any administrators.
* Please allow 5-7 business days for the processing and distribution of the certificate, however most certificates will arrive in 1-2 business days.

If one (or more) of the pool facilities you are using requires a certificate of insurance, the club administrator can log in to his/her personal USAWP Membership account to request a certificate. After logging in, scroll over the "Club Admin Area" tab on the left column and click on the "Practice Insurance" option.
NOTE: Please be sure to fill out the form completely. All pool locations should be listed - even if insurance is not required.
For a facility/organization requiring additional insured listing, please be sure to include the organization/facility and address in the Additional Insurance Info section.
For a facility/organization requires an endorsement, be sure to list all information (including any specific wording) in the special instructions section. Please note: For questions regarding insurance, please email firstname.lastname@example.org. To request insurance for a league, tournament, camp or clinic, you must first submit an event sanction.
